Are you or a loved one eligible for hospice care?

To be admitted to Ease of Mind Hospice Care, patients can meet the criteria based on their diagnosis or symptoms. You have the option to either contact us directly via email and/or phone call OR complete the referral form available on our website under the "Make A Referral" tab.

Some possible signs that a person may be ready for hospice include (but not limited to):

experiencing frequent or recurring infections

rapid decline in health despite aggressive medical treatment

frequent hospitalizations or trips to the emergency room

uncontrolled pain, nausea, or vomiting

increase in the amount of time spent sleeping

loss of urinary or bowel control

chest congestion and/or rapid breathing

decrease in food and fluid intake

changes in body temperature (fever or feeling cool to the touch)

restlessness or repetitive motions

inability to perform daily tasks without assistance

It's important to note that each person's situation is unique, and a healthcare professional can help determine if hospice care is appropriate.
